Description
Momentum Healthcare Staffing is seeking a PMHNP for a behavior health practice located in Phoenix, AZ.
Requirements:
Specialty: Behavioral health/SMI
Education: PMHNP
License: Arizona and DEA
Certification: Board Certified
Patient base: 2 openings, one with children and one with adults
Facility Details:
Dates/ Schedule needed: 25-30 hours per week
Patient Population: 20 minute medication appointments and 1 hour psychiatric evaluation appointments.
Salary Details: base pay is 150k with extremely attractive quarterly bonuses.
