Description
Full Time Direct Hire RN's Wanted. Fitchburg MA. Must have MA RN License. 40 hours a week, 4 k sign on Bonus, Salary $60,000-$75,000.
These are 100% outpatient .. no nights /weekends
In collaboration with the provider (PCP) the RN Care Coordinator helps to identify, assess and coordinate the care for complex care patients. These patients are defined as those at high risk for health deterioration, sentinel events and /or poor outcomes. The medical facility is out patient and 30-40 beds.

Birthday –One of the five (5) Personal days is issued to celebrate the employee’s birthday. This day may be taken any time during the year.
Sick Time – Employees accrue up to a maximum of 40 hours paid sick time per fiscal year. Eligible employees may carry over unused sick time up to a maximum of 720 hours.
RETIREMENT – TAX DEFERRED ANNUITY ACCOUNT (401K) – 401K Retirement Accounts with company contribution match for ALL employees.
