An established medical group in west central Illinois is looking for a Geriatric Nurse Practitioner or Physician Assistant to join their team. Schedule is full time working in an outpatient setting. Ideal candidate will have experience in adult/geriatric medicine as an advanced practitioner. As an extension of the physicians, provider can expect to manage post-acute issues, coordinate care transitions, provide direct patient care and help cover 5 local nursing homes.
