The
Life of an HCA Nurse
The Heart Failure Nurse Navigator has responsibilities which
include assessing, planning and directing appropriate care in Cardiology and
creating an environment that promotes, stimulates and fosters personnel growth
and satisfaction. They have high-level
critical thinking skills, vast clinical knowledge, and are able to stay calm
under pressure.
About
this unit: - Work schedule - Monday - Friday, No weekends.
- Procedures done in the cath lab/hybrid range from but not limited to diagnostic heart caths, interventional heart caths, TAVR, TMVR, watchman and cardiomems.
- The lab also works with vascular surgeons and performs many peripheral procedures.
- Specialized area with high acuity and close collaboration with cardiologists
Responsibilities: Responsible for
coordinating all aspects of the delivery, facilitation and improvement of high
quality clinical care of the heart failure patient. The HF Coordinator serves as an educator,
consultant, coordinator, researcher and program planner/implementer. The role includes assessing, educating and
promotion of disease self-management capabilities of the patient and their
families. Works as a collaborator and liaison between heart failure patients,
case management, families, nurses, physicians, administration, and other health
care services and community service providers. Schedules appointments for follow up, schedules prior to or at discharge
patient’s follow up care with provider within one week and conducts phone
follow up within 72 hours to assess patient’s needs post discharge, reinforce
discharge instructions and perception of care. The Coordinator will work within the facility’s clinical practice
guidelines, policies and procedures, and will follow recommendations and
guidelines set forth by the Heart Failure Committee and the program’s medical
directors. The Coordinator will serve as
the Heart Failure Coordinator for Accreditation and organize, facilitate and
implement activities for recertification processes under the direction of the
Director of Cardiovascular Services. Participates in Performance Improvement program in the development of
and compliance with treatment guidelines, assists with educational programs
related to heart failure care. Serves as
resource for clinicians regarding heart failure care, guidelines and core
measures adherence, and clinician competency to provide heart failure
care. Assists with Heart Failure Core
Measures rounds and documentation completion.
